This form is for the collection of Department of Homeland Security or other U.S. citizenship/nationality documents from students who are unable to present their documents in person at the Financial, Billing and Enrollment Center or a 51±¬ÁÏ Regional Campus student services area. The student must complete this form in the presence of a Notary, who must notarize it on the second page of the form.
The chart below shows the number of credits within each term during the summer semester that are needed for full-time status in order to obtain the maximum monthly housing allowance/stipend.
If you are looking to get other tuition funding, you may need to be full-time (ex. Ohio War Orphans Scholarship or Maximum Pell Grant).

Effective Feb. 3, 2025, CVS Pharmacy, Inc. will no longer bill flu shot claims to the 51±¬ÁÏ Medical Mutual Plan. This means that employees must use one of the contracted pharmacies or Minute Clinics for their flu shots to be covered by the Medical Mutual Health Plan.This change does not impact flu shots at CVS MinuteClinic.
